The frst thing to realise is that Paediatrics is a lot of fun! (You will notice that the most important learning points are in bold throughout the book, the frst one is probably the most important of them all.) I stopped being a GP after 5 years as a partner not because I was fed up with general practice, but because I loved being a doctor to children. Paediatrics is fun, because children are fun. You can blow raspberries at them and get paid for the time spent doing so. Paediatrics is also rewarding because it is very much how medicine used to be. Do you remember what you imagined yourself doing when you decided to be a doctor? Was lowering the nation’s cholesterol a major incentive for you to become a doctor? Well, the great news is that most of paediatrics still involves treating (wait for it….) illness. What’s more, the patients almost always get better.
Finally, before you read on, I would like to point out that I have not forgotten how little time you have in the general practice (GP) surgery or the emergency department (ED) to assess problems and make decisions. The guidelines I have written are as detailed as I can make them without making them cumbersome, but hopefully simple enough to use once you feel that you have a good understanding of the problem. By spending a little extra time with these problems when they present, I believe that you will avoid unnecessary investigations, ensure that you identify genuine problems earlier, and generally feel more satisfed with managing these conditions. I have written this book with the hope that you can use it both as a textbook at your leisure and as a quick reference book when under fre. For this reason there is a combination of detailed explanations and quick reference lists and flowcharts. I also recognise that the modern clinician has guideline and information overload. What follows tries to cut through to the heart of what is important for each presentation. All content is designed to be used by clinicians who are able to make up their own mind about what is best in any given circumstance. So may these pages help you to enjoy practicing medicine, and to show your paediatric colleagues that they don’t really have a monopoly on being a know-it-all.
