The care of the patient who presents with surgical concerns on an unexpected basis was once solely in the domain of the general surgeon who also practiced in the scheduled realm. While that remains the case in many hospital systems, it is increasing being replaced by care delivered by surgeons who primarily, if not only, encounter these patients on an urgent basis. This evolving paradigm has caused us to transition from pairing the patient who has a certain clinical problem with a surgeon who has a certain clinical skill set to pairing the patient with the person who happens to be at the facility at that time.
